OnVerra Health is a New Jersey-licensed Home Health Care Service Firm (NJ Entity ID: 0451355434) providing personal care, homemaker, and companion services. Our website provides information about our services, allows prospective clients to request care, enables job applicants to apply for positions, and provides authorized users access to our care management platform.

2. Website Use

You agree to use our Site only for lawful purposes and in accordance with these Terms. You agree not to:

• Attempt to gain unauthorized access to any part of the Site, other accounts, or computer systems

Employee and caregiver accounts are subject to additional terms as outlined in employment agreements and company policies.

4. Care Inquiries and Applications

Information submitted through our care inquiry form (/get-care) or job application form (/apply) is used solely for the purpose of evaluating your request. Submitting a care inquiry does not guarantee services, and submitting a job application does not guarantee employment. All care services are subject to eligibility screening, insurance verification, and clinical assessment as required by New Jersey regulations.

5. Electronic Signatures

Our platform supports electronic signatures for intake documents, service agreements, and other forms. By using our electronic signature feature, you consent to conducting these transactions electronically and agree that your electronic signature carries the same legal weight as a handwritten signature under the federal ESIGN Act and New Jersey's Uniform Electronic Transactions Act (UETA).

6. Privacy and HIPAA

Your use of our Site is also governed by our Privacy Policy. Protected Health Information (PHI) transmitted through our platform is handled in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
