Services We Provide
OnVerra Health provides home health care services in New Jersey, including personal care (bathing, dressing, grooming, toileting assistance), companionship (social engagement, escort to appointments, errands), and homemaker services (light housekeeping, laundry, meal preparation). All care is provided by Certified Home Health Aides (CHHAs) under the supervision of a Registered Nurse.
Patient Rights
As a patient of OnVerra Health, you have the right to:
- Be treated with courtesy, dignity, and respect at all times
- Receive complete information about your care plan, services, and any changes to your care
- Participate in planning your care and making decisions about your treatment
- Refuse any treatment or service without penalty
- Have your personal health information kept private and confidential in accordance with HIPAA
- Access your medical records and request copies
- Voice grievances or complaints without fear of retaliation
- Receive care without discrimination based on race, color, religion, sex, national origin, age, disability, or sexual orientation
Patient Responsibilities
To help us provide the best possible care, we ask that you:
- Provide accurate and complete health information
- Follow your agreed-upon care plan to the best of your ability
- Notify us promptly of any changes in your condition or concerns about your care
- Treat our staff with courtesy and respect
- Maintain a safe environment in your home for care delivery
Complaint Procedures
If you have a concern or complaint about your care, please contact our office first. We will investigate and respond within 5 business days. If you are not satisfied with our response, you may contact the New Jersey Division of Consumer Affairs:

Emergency Procedures
Our caregivers are trained in basic emergency procedures. In the event of a medical emergency, your caregiver will take the following steps:
Call 911 immediately for any life-threatening emergency
For non-emergency concerns outside business hours, call our main office number. A staff member will return your call as soon as possible.
Fees & Billing Information
OnVerra Health accepts Medicaid managed care (MCO) plans and private pay arrangements. We do not balance-bill patients for services covered by their insurance plan. For private pay services, rates are discussed and agreed upon before care begins. You will never receive unexpected charges for authorized services. Contact our office for current rate information and insurance plan participation.
Privacy Practices
OnVerra Health is committed to protecting your personal health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA). We collect only the information necessary to provide your care, share it only with those involved in your treatment and as required by law, and maintain appropriate safeguards to protect your records.
