Under the New Jersey Data Privacy Act (N.J.S.A. 56:8-166.4 et seq.), you can ask us to delete, export, or correct personal information we hold about you, or opt out of our marketing communications.
This is for non-medical records only. Requests about client care records, such as a copy of your record, go to our Privacy Officer at (908) 718-1948 or info@onverrahealth.com and are handled under the privacy notice in your admission packet.

Our online privacy-request form is closed. Email or call us with your request and we will handle it directly. Tell us what you would like us to do — delete, export or correct your information, or remove you from our marketing list.
